Patient case files include statistical data sheets; physical and psychiatric examination results; anamnesis; laboratory results; commitment papers; nursing records; staff meeting summaries; progress notes; death certificates or discharge records, and correspondence with family and friends. Statistical data sheet usually includes patient picture; name, age, sex, marital status, occupation, race, religion, birthplace, education, and residence; mental and intelligence quotient test results; parents' birthplaces; numbers of brothers and sisters; and other family history.

The Illinois General Assembly instructed the Board of State Commissioners of Public Charities to plan the Illinois State Colony for Epileptics (1899) but construction and operational funds were not appropriated until 1913. Construction began on the Dixon site under Board of Administration supervision (1909-1917).
